Exploring GP-Led Emergency Care: Insights from Norway

The growing pressure on emergency departments (EDs) worldwide has led healthcare systems to explore innovative solutions to alleviate crowding and improve patient care. In Norway, a ground-breaking approach is being tested: a General Practitioner (GP)-led emergency primary care response unit (EPCRU). This initiative aims to streamline emergency care, enhance resource allocation, and optimise patient pathways. An observational study conducted across two Norwegian municipalities has provided insightful findings on the impact of this innovative system.

Enhancing Emergency Care with GP-Led Units

In the study, the EPCRU was tasked with responding to a variety of emergency situations. Over the course of a year, from April 2023 to March 2024, the unit was dispatched on 2,950 missions. These missions covered a wide range of conditions and severities, demonstrating the unit's capability to handle diverse emergency scenarios. Significantly, the EPCRU was the first responder in 44.1% of these cases, underscoring its pivotal role in providing immediate care.

Efficient Resource Allocation and Patient Flow

A key finding of the study was the EPCRU's ability to effectively triage and manage patients on-site. In 44.1% of cases, patients were treated at the scene, eliminating the need for hospital conveyance. This not only reduced the strain on hospitals but also expedited patient care, allowing for timely interventions. Interestingly, there was a notable mismatch between telephone triage and on-site assessment; 34.2% of cases initially categorised as 'Acute' were reassessed as 'Yellow' upon arrival by the Rapid Emergency Triage and Treatment System (RETTS).

Impact on Care Trajectories

The EPCRU's influence extends beyond immediate care. It alters traditional care pathways by facilitating both non-conveyance and direct admissions to the ED when necessary. This flexibility in patient management offers a more tailored approach, ensuring that patients receive the right level of care promptly.
